Care Coordination
Our Care Coordinators are advocates, helping patients navigate the healthcare system. Your care coordinator is available to answer questions, help with referrals, and assist with follow-up appointments. They can also help you access additional support and social services you may need.
SNH AHEC
Southern New Hampshire Area Health Education Center is a program of Lamprey Health Care and is an accredited provider of continuing education. We strive to provide learning opportunities for health professionals and community members on a wide variety of topics.
Seacoast PHN
The Seacoast Public Health Network is a program of Lamprey Health Care with a mission to strengthen public health partnerships in emergency preparedness, community health, and substance misuse prevention in order to better serve our communities.
