Requesting Your Medical Records

Medical Records Online

Many medical records are available through the Lamprey Health Care Patient Portal, your secure online access to your information. If you have not already done so, you will need to register and create an account. Once you’ve done that, you will have safe, confidential access to your medical records.

Paper Medical Records

If you need a paper copy of your records, we require a completed medical records release form.

Faxed or mail forms to our office. The form lists the return mailing address and fax number. Please allow 2-4 weeks for the preparation of the records.

Do you have a Personal Representative?

You can authorize other people to be involved in your care. These people are personal representatives.

To authorize health center staff to speak with a spouse or another individual regarding your health care, we will need to have this form completed. This includes conversations such as rescheduling appointments or questions on billing issues.

Interpretation Services & Bilingual Staff

The majority of our Nashua Center staff speaks more than one language, with nearly half of our staff being fluent in Spanish or Portuguese. Staff is available through all aspects of your visit to help you communicate with your health care team and make you feel comfortable about your medical care.

To help with languages other than Spanish and Portuguese, we offer free onsite language interpretation by a professional interpreter. To ensure that your language needs are met, please let us know when you make your appointment that you require language services. This way, we can be sure someone is available to interpret at the time of your visit. If we don’t have access to an interpreter who speaks your language, we can utilize a Language Line interpretation service to assist with your visit.
