Our Integrated Behavioral Health Program makes it easier to access behavioral health resources and support for both adults and children.
Your care team may include:
- A behavioral health clinician
- A medical provider specializing in psychiatric medications (if needed)
- Your primary care provider
- Care Coordinators who connect you with community resources
Everyone works together through shared decision-making and electronic health records to provide the best care for you and your family.
During your primary care visit, your provider may:
- Consult with the Behavioral Health Team
- Invite you to meet with a clinician as part of your visit
- Arrange a future behavioral health appointment
This flexible, team-based approach ensures care is tailored to your needs.
Behavioral Health
You can schedule an appointment directly with the Behavioral Health Care Team or through your Primary Care Provider. The Behavioral Health Care Team is here to help with:
- Depression
- Life changes and challenges
- Anxiety
- Substance use
- Coping with chronic disease
For more complex Behavioral Health issues, our team will assist you with referrals to appropriate care facilities. Our goal is to improve your access to Behavioral Health Services in a safe and comfortable environment. You’ll receive the care you need through integration with your primary care medical home.
