Lamprey Health Care Earns Four National Quality Recognitions, Including HRSA Gold Status and NCQA Medical Home Designation

Lamprey Health Care has earned national recognition for quality, innovation, and patient-centered care, receiving three 2026 HRSA Community Health Quality Recognition badges and achieving NCQA Patient-Centered Medical Home (PCMH) Recognition. This leads to better health outcomes for thousands of NH residents.

The organization received:

  • Health Center Quality Leader – Gold
  • National Quality Leader – Behavioral Health
  • Advancing Health Information Technology (HIT)

Together, these recognitions place Lamprey among the nation’s top-performing community health centers and reflect measurable excellence in clinical outcomes, behavioral health integration, care coordination, leadership, and the use of technology to improve patient care.

The Health Center Quality Leader – Gold designation recognizes Lamprey’s performance on key quality measures and places the organization among the top 10% of highest-performing health centers nationally.

The National Quality Leader – Behavioral Health award highlights Lamprey’s success in integrating behavioral health services into primary care, while the Advancing HIT badge recognizes the effective use of data, electronic health records, and technology-driven strategies to improve care coordination, patient engagement, and clinical performance.

Adding to these achievements, Lamprey Health Care has also earned Patient-Centered Medical Home Recognition from NCQAone of the nation’s most respected quality accreditation organizations. The PCMH model is designed to strengthen relationships between patients and care teams while improving access, care coordination, communication, and overall health outcomes. The recognition demonstrates Lamprey’s commitment to delivering care that is coordinated, team-based, accessible, and centered on each patient’s needs.
