Founded in 1971 by a group of concerned citizens, Lamprey Health Care began as an information and referral service with the goal of connecting people to health services. The group quickly realized the need for more community-based service providers. In 1972, the group started the Senior Transportation program, and in 1973, the first primary care office opened in Newmarket, NH. In 1981, the organization added a location in Raymond, NH. Over the decades, the Newmarket and Raymond centers moved and expanded facility space to meet the growing community needs. In 2000, the Nashua Center was added, located in the downtown area.
As we grew geographically, programs also expanded. Prenatal care was added in the 80s and school-based dental care in the 90s. In the 2000s, we added integrated behavioral health, substance use disorder services, medication-assisted therapy, and the Nurse Practitioner Fellowship Program. We have also grown our community collaborations. In 1997, we established the Southern NH Area Health Education Center, and in 2015 we became the fiscal agent for the Seacoast Public Health Network.
As we approach the future, we look forward to adding to our history of service.
Lamprey Health Care is pleased to be awarded the Community Health Quality Recognition (CHQR) - Health Center Quality Leader (HCQL) SILVER LEVEL badge for 2024 from the Health Resources & Services Administration (HRSA.) This is awarded to the top 20% of all health centers nationally achieving the best overall clinical quality measurement performance.
Additionally, Lamprey Health Care was one of only two health centers in New Hampshire to achieve this recognition, and one of only three health centers in New Hampshire to achieve silver or bronze. (There were no gold awarded this year.)

Lamprey Health Care, at its Newmarket, Raymond, and Nashua offices, has been awarded recognition by the National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home Program.
The Patient-Centered Medical Home (PCMH) is a model of care that puts patients at the forefront of care. PCMHs build better relationships between patients and their clinical care teams. Research shows that PCMHs improve quality, the patient experience and staff satisfaction, while reducing health care costs. Practices that earn recognition show that they have made a commitment to providing quality improvement within their practice and a patient-centered approach to care.

NCQA is a private, nonprofit organization dedicated to improving health care quality. NCQA accredits and certifies a wide range of health care organizations. It also recognizes clinicians and practices in key areas of performance. The NCQA Patient-Centered Medical Home program reflects the input of the American College of Physicians (ACP), American Academy of Family Physicians (AAFP), American Academy of Pediatrics (AAP) and American Osteopathic Association (AOA) and others. It was developed to assess whether clinical practices are functioning as medical homes and recognize them for these efforts. The NCQA Patient-Centered Medical Home standards emphasize the use of system, patient-centered, coordinated care that supports access, communication and patient involvement.

For more than 50 years, Lamprey Health Care has been a leader in community health, both within the state and across the country. Our Board of Directors, Executive Team, and Clinical Team have all won numerous awards for their dedication and excellence in the field of health care.
Lamprey Health Care has been a recognized Patient-Centered Medical Home (PCMH) since 2013. In 2024, we once again received PCMH recognition from the National Committee for Quality Assurance.

Raaga Devineni Equity and Justice Award
Paula Smith, Director of Southern NH Area Health Education Agency, was awarded the Raaga Devineni Equity and Justice Award by the NH Public Health Association in 2023. This award was established in 2021 in memory of the first coordinator of the COVID-19 Equity Task Force and is presented to an individual who has demonstrated passion, commitment, and dedication to promoting health, and economic and social justice for everyone in New Hampshire.
NH Healthcare Heroes Recognized
The NH Healthcare Heroes effort recognized the dedication, courage, and commitment of 7 professionals in the healthcare community. Winners were honored during pinning ceremonies with each of their employees. Stacy Dionne, Clinical Nurse Manager at our Raymond Center, was the Seacoast Region Winner. Congratulations, Stacy!
Hunt Blair Leadership Award
Vasuki Nagaraj, MD, was recognized by Bi-State Primary Care Association with the Hunt Blair Leadership Award. This award recognizes the potential every individual has to improve lives, as demonstrated by the brilliance, tenacity, and vision of Hunt Blair. The award also recognizes Dr. Nagaraj’s excellence in improving the care and health outcomes of patients.

Lamprey Health Care was honored by the Newmarket Business Association in recognition of our role as a community service provider in the town of Newmarket.
The Community Partner Award
Lamprey Health Care was honored by the Raymond Coalition for Youth (RCFY) for playing an instrumental role in the success of RCYF to promote substance abuse awareness and prevention programs.

Sandra K. Pardus was recognized by Bi-State Primary Care Association for her outstanding contributions to assure access for vulnerable populations and tireless engagement in activities well beyond the scope of her day to day job.
The Business of the Year Award
Business NH Magazine selected Lamprey Health Care as Business of the Year in the Health Care category in recognition of business excellence in the state of New Hampshire.
New Hampshire Outstanding Clinician Award
Bi-State Primary Care Association recognized Jeremy James, MD, and Sarah Zachos, MD, Family Physicians in the Raymond Center, for exemplary leadership skills and their impact on the health of the community they serve.
McBean-Little Diabetes Educator of the Year
Tracy Tinker, Certified Diabetes Care and Education Specialist (CDCES), was awarded the McBean-Little Diabetes Educator of the Year award in 2022, voted on by her peers, and given in recognition of her outstanding contribution to diabetes care and education in New Hampshire.
Non-Profit of the Year
In May 2022, Lamprey Health Care was recognized as the Exeter Area Chamber of Commerce Non-Profit of the Year.
Community Service Award
Cynthia Giguere-Unrein, Director on Lamprey Health Care’s Board, was honored by Bi-State Primary Care Association for her considerable volunteer efforts and impact on the vulnerable populations of New Hampshire.

Through the State of New Hampshire, Lamprey Health Care had a grant to provide dental care to patients with diabetes through a partnership with Greater Nashua Dental Connection.
Lamprey Health Care was awarded a grant to help fund our Early Childhood Development Program, This allowed us to expand availability of our services provided by an early childhood development expert. Children ages 0-5 receive recommended developmental screenings and their families are assisted with accessing appropriate follow-up services.
